The most common cause of this is that the device has been flashed with a debug build from Particle Workbench, such as from Particle: Flash application from debug (local). Some versions of Boron Cloud Debug are also debug binaries.
In any case, best way to recover from this is by USB. In DFU mode (blinking yellow):
particle update
particle flash --usb tinker
This device is in a remote location and I am only able to use OTA for updates.

Is this statement valid for the last flash event, or any time in its life cycle? I ask because of my statement below.

I can’t explain how it may have gotten into that state in that case. You should create a support ticket and include the Device ID of the device in question so they can check the cloud logs for the device.
